Before your child can start at any Florida child care facility, you will need to have a current immunization (Form DH 680) and physical (Form DH3040). These can be obtained from your pediatrician, or if moving, the health department.

In order to have your child put on our wait list, complete the enrollment form above. This form can then be emailed to admin@birdsnestschool.com, dropped off in person or snail mailed our way!
