Enrollment Agreement Enrollment AgreementPlease read each of the statements on the enrollment agreement. Submit the form at the end. Name * First Name Last Name Child's Name * Registration Fee I understand a one-time, non-refundable registration fee of $75 should be paid in advance to enroll my child. I have read and agree to the policy Supply Fee I understand an annual, non-refundable, supply fee of $100 will be paid on my child's start date and then in August for each continued year. I have read and agree to the policy Diapers/Wipes I understand that at the beginning of each month I will need to supply my child's room with enough diapers and wipes to last the month (about one box of diapers and three containers of wipes). I have read and agree to the policy Tuition Rates I understand rates and fees are subject to change. The Bird's Nest will provide written notification before any changes take place. Please be aware of your current program weekly rate and select it below. Chickadees $245 Toucans $230 Robins $220 Starlings $210 Tuition Payments Tuition is to be paid on the first day of the week (Monday) for weekly payers and the first day of the month (1st) for monthly payers. Payment can be drawn automatically, online or in person. I have read and agree to the policy Late or Unpaid Tuition Any payment not received in FULL on the due date will be assessed a $35 late fee. If an account is delinquent for more than two weeks, you will be asked to withdraw your child. In the instance you are asked to withdraw your child The Bird's Nest cannot guarantee your child's space will be held or available at a later date. I have read and agree to the policy Late Pick Up I understand I must pick my child up at or before 6:15pm. I will be billed $1 for every minute after closing with my child still present. For any students still at school an hour after closing, local police will be notified. The state of Florida also requires that the Department of Children and Family Services be notified. I have read and agree to the policy Discounts The Bird's Nest offers a 5% sibling discount and a 5% Teacher/Military/First Responder discount (with valid ID). I understand I am eligible for this discount. All discounts will be applied to the oldest child's tuition rate. I have read and agree to the policy Returned Checks/Declined Cards I understand a processing fee of $50 will be assessed on my account for all checks which are returned for any reason or any credit/debit card that is declined for payment. This is in addition to any fees you may incur from your banking institution. If more than two checks are returned in a six-month period, you will need to pay with a different method. If a credit/debit card is declined more than 2 times in a six month period an alternate payment method will be required. I will be responsible for the principal balance and all fees on my account. I have read and agree to the policy Illness I understand I will be notified should my child become sick during the day. I will make arrangements to have my child picked up from the center within the hour. I understand that if anyone in my family is planning to test for COVID, or has COVID like symptoms, I will notify The Bird's Nest immediately. My child will be re-admitted to school following the guidelines in the Parent Handbook. I understand tuition is NOT prorated for missed days due to illness. I have read and agree to the policy Wellness Policy Students must be symptom free for at least 24 hours before attending or returning to school. If students are sent home sick, they will miss the following day of school. Symptoms may include but are not limited to a fever (100.5 F or higher), pink eye, diarrhea, vomiting, rash, lethargy, raspy cough (hoarse like), etc. Some occasions will require a doctor's note to return to school . Health and safety for our students is our top priority. I have read and agree to the policy Medications Any medications kept on hand for students at The Bird's Nest must be in original packaging (box included), be labeled with the students first and late name, within expiration date and must have a medications form on file for each type of medication. This includes but is not limited to prescriptions, pain relievers, diaper cream, bug spray, and sunscreen. I have read and agree to the policy Choking Hazards Please be mindful of choking hazards when sending in food, snacks or treats. Please be sure to cut grapes, hotdogs, berries and other items correctly for your child's age. I have read and agree to the policy Health Forms I understand my child must have a current physical and immunization record on file. I understand I will be notified when my child's dates are about to expire. I understand that my child CANNOT attend school with expired health forms. Expired health forms will require your child to be absent from The Bird's Nest until new forms are completed and turned into the Director. I have read and agree to the policy Authorized Dismissal Release I understand that anyone picking up my child must have PRIOR written permission or be on the emergency contact of my child's registration form. I must notify The Bird's Nest in WRITING of any dismissal changes. Please include the first and last name, relationship to child, date of pick up and a phone number for anyone NOT on the child's emergency contact pick up. All persons must present a government issued identification and have proper child seats. I have read and agree to the policy Vacation/Flex Week Rates Full time student accrue time to be used for vacation (a full free week) or flex rate (two weeks through the year at half rate). Vacation or Flex time may be used AFTER your student has been enrolled for three months. The Bird's Nest requires two weeks written notice ahead of your vacation or flex rate weeks. (Email is ok!) These are not cumulative hours and reset every August. The cannot be used if your student is withdrawing. They cannot be used for individual days. I have read and agree to the policy Parent Hand Book I have received The Bird's Nest Parent Handbook. I understand it contains the policies and procedures of the school. I agree to comply with the policies and procedures set in place. I understand any changes will be given to parents as soon as they are decided on. I understand the Handbook is also always available online. I have read and agree to the policy Withdraw/Dismissal I understand I must provide a two weeks written notice to withdraw from my child's program at The Bird's Nest. I understand all tuition and fees are due for the final two weeks of the program regardless of my child's attendance. I understand that re-enrollment is at the discretion of the director and based on availability in the room.
